Appointment Policy
Appointments at Mayfair Animal Clinic are by scheduled appointments only. We do not offer walk-in services. Please call 773-478-1868 to book an appointment.
Medication refill requests require a minimum of 24 hours notice.
Patient Arrival Policy
Even though we make every effort to make our patients feel comfortable during visits, they may be uneasy about new people, new surroundings and other pets. For your protection, and that of others, all dogs must be on a leash and properly controlled while in the waiting area or exam rooms.
All cats must be presented in an appropriate cat carrier or on a leash.
For the safety of all animals in our care, we require that rabies vaccinations be up to date.
If you are more than 15 minutes late to your appointment, you will be asked to reschedule.
Payment Policy
We require full payment at the time that services are rendered. For your convenience, we accept VISA, MasterCard, Discover, cash and personal checks. Care Credit is also available.
Plan For Your Visit
We offer free off-street parking on Harding, south of NAPA.
If your pet is having urinary issues, please do not allow them to urinate on the way into the clinic. We may need this urine as a sample to send to the lab.
When you arrive at Mayfair Animal Clinic PC, you’ll be asked to sign in at the front desk with your pet. Once an exam room is available, the veterinary technician will then lead you to the room and take a brief history. The veterinary technician will then update the doctor with the history. The doctor will then ask some revised history questions that may be specific to your individual pet’s needs.
Depending on the case, your pet may need diagnostics such as bloodwork, a urinalysis, or radiographs (x-rays) to help determine the cause of why they are not feeling themselves. Some diagnostics require several days to get results, such as biopsies. Diagnostic tests may have to be re-done in the future to check progress or if they are inconclusive.
What information to bring:
- What medications your pet is currently receiving including the amount (mg) listed on the medication
- What food (including brand) your pet eats
- What treats (including brand) your pet eats
- What types of toys or bones your pet can play with or chew at home
- What exposure your pet has had to other animals (wildlife, daycare, dog-parks, groomers, etc)
- Any vaccine or medical history performed by a veterinarian not at Mayfair Animal Clinic. This information would ideally be emailed to [email protected] prior to the visit.
